Express Employment Professionals filed a data breach notice Sept. 13 with the Texas attorney general’s office.

The type of information affected includes individuals’ names, Social Security numbers, driver’s license numbers, financial information (examples could include account numbers and credit or debit card numbers), medical information and health insurance information.

Those affected have been contacted, according to the notice. It also indicated 5,941 Texans were affected.
